Paratransit Program Application Forms

Measure B paratransit fund recipients are required to submit to the Alameda CTC one electronic version of two documents for the annual program application submittal:

  1. Paratransit Program Application (PDF) - due April 8, 2011
  2. Paratransit Program Table 1-2 Attachment - due April 8, 2011

Electronic submission: Save the online PDF form to your hard drive with your agency name and date in the file name (e.g., Albany_FY1112_Paratransit_Program_Application_040611.pdf). You can start work on the PDF and finish it later; simply save it to your hard drive. Also, complete the Table 1 and Table 2 workbook and include your agency name and date in the file name. Submit one copy of both the PDF and Table 1 and Table 2 Attachments via email by April 8, 2011.

Mid-year Paratransit Program Reporting Forms

Measure B paratransit fund recipients are required to submit to the Alameda CTC one electronic version of two reports for fiscal year 2010-2011 mid-year reporting:

  1. Mid-year Paratransit Program Report (PDF) - due March 18, 2011
  2. Mid-year Table 1 Attachment - due March 18, 2011

Electronic submission: Save the online PDF form to your hard drive with your agency name and date in the file name (e.g., Albany_FY1011_Paratransit_Program_Midyear_Report_031611.pdf). You can start work on the PDF and finish it later; simply save it to your hard drive. Also, complete the Table 1 Excel workbook and include your agency name and date in the file name. Submit one copy of both the PDF and Table 1 Attachment via email by March 18, 2011.

End-of-year Compliance Reporting Forms

Measure B fund recipients are required to submit to the Alameda County Transportation Commission (Alameda CTC) one electronic and one hard copy version of an audit report, a compliance report and a Table 1 Attachment for end-of-year compliance:

  1. Audit - due December 27, 2010
  2. EOY Program Compliance Report (PDF) - due December 31, 2010
  3. Table 1 Attachment (Excel) - due December 31, 2010
Note: To save a copy of the form and attachment on your desktop that you can continue to update before you submit the final form, right click on either of the above documents and depending on your browser, choose "Save link as" or "Save target as" to save a copy on your hard drive. Add to the existing file name a designator for your agency/jurisdiction. (You can also save a copy of the form and attachment after you click through to them above, but the approach varies according to your browser.)

This year, we are requesting that you email all three documents in one email to Carol Crossley at grants@actia2022.com and mail in hard copies before the due date.
